Note: you cannot currently list a “Country” when completing the “Emergency Contact” section on our forms. Please list an emergency contact in the United States.

 Complete and print online form DS-11. Please do NOT sign it!  If the applicant does not have a U.S. Social Security Number, please complete and submit this form with the passport application.
BOTH PARENTS’ VALID IDS: Original and 1 copy
BIRTH CERTIFICATE Original and 1 copy of the French birth certificate
PHOTO One color 2×2 inch photo. Click here for photos requirements
 A RETURN ENVELOPE Include either a prepaid Chronopost for France, or Collissimo, or a Lettre suivie 500g size A4 only, for the return of your passports.
 PAYMENT You can pay the fee of 115 USD (or the Euro equivalent) at the time of your interview in cash or credit card (the credit card holder must be present).
