Note: you cannot currently list a “Country” when completing the “Emergency Contact” section on our forms. Please list an emergency contact in the United States.
Complete online and print forms: Form DS-11 and Form DS-64 (If your passport was lost or stolen)
If your current passport has less than 6 months validity, you only need to complete Form DS-82.
Please do NOT sign forms DS-11, DS-64, or DS-82.
If you never had a U.S. Social Security Number, fill out and submit this form.
If you do not remember your U.S. Social Security Number, please email the Federal Benefits Unit before applying for the passport.
Tip: Forms may be completed, and photos taken onsite at the embassy.
Tip: If you have had an identity document lost or stolen, go to the Dept. of Justice’s suggestions for information on protecting yourself from identity theft.

Adults over 16: Copies or originals of any/all photo IDs you still have;
For minors under 16: Both parents and the child must be present. Both parents must consent to passport renewal. If only one parent can come, please click here (look at step 7).

PAYMENT
Pay the fee at the time of your interview, in cash or by credit card (the credit card holder must be present with a valid Government photo ID).

b. Lost/stolen/damaged passport and I am NOT travelling within the next 7 days: For the return of your passport by mail you will need a prepaid self-addressed Chronopost for France, or Collissimo, or a Lettre Suivie 500g size A4 only. PROCESSING TIME: TWO WEEKS
