Questionnaire for Medical Professionals for Consultations with U.S. citizens in France

The Office of Medical Services of the United States Department of State requires that information and credentials of all consultants be retained at each overseas post.  This applies to all medical, mental health and dental professionals whose names appear on the above-mentioned list.

The U.S. State Department requires regular reviews of credentials of medical providers listed on the Embassy website pages.

Generally, the lists are revised triennially with interim addendums as needed, to ensure that local physicians are still practicing in good standing within the consular district and are willing to treat patients from the United States.

The U.S. State Department requires regular reviews of credentials of medical providers listed on the Embassy website pages.

In compliance with this directive, please provide the following:

  • copies of diplomas or other medical credentials, and updated currently expired Florida state license;
  • medical license(s) (attach copy), or copy of registration with conseil de l’ordre des medecins, dentistes etc.;
  • the attached affirmation which must be signed and dated;
  • A statement of competency in English, i.e., studies in the United States, published studies, practice using English, staff members or support personnel who also speak enough English to make non-French-speakers comfortable.

Inclusion of a physician’s name on the list is exclusively within the Embassy’s discretion, and the Embassy may remove a person from the list at any time, and may decline to provide a reason.

Our regulations prohibit us from recommending any particular doctor, dentist, or facility.  We simply make this information available to interested U.S. citizens without recommendation.
