Step A: Complete and submit application Form DS-5504 available using the online passport wizard. Be sure to answer all questions accurately as any mistakes may result in a delay. Print one-sided pages only, sign and date.
- Social Security Number: If the applicant does not currently have an SSN then he/she should fill in the passport application SSN box # 5 with zeros. The applicant MUST also fill out and sign the SSN Declaration (PDF-80 kb) and submit it with the passport application. Absence of the form will result in a delay of passport adjudication.
Step B: Submit current valid passport;
Step C: Provide one recent photo: U.S. standard size passport photograph for biometric passports.
Note: French photo booth photographs are NOT acceptable for U.S. passport issuance. All photos must have a white background. Do NOT staple or paperclip the photo to your application.
Step D: You MUST provide a return envelope. Please provide ONE of the following:
- A Collissimo (“pret-a-envoyer”) or Chronopost (“pret-a-expedier”). One applicant can use the 1 kg (the smallest available) envelope and multiple applicants can use the same envelope, as long as you allow for 500g per applicant. Please keep top copy of delivery information with bar code and tracking number for your records (one envelope can contain up to 6 passports).
Step E: Submit documentary evidence such a court order, marriage or divorce certificate, or other satisfactory evidence to support a change of name. Only originals or certified copies of these documents will be accepted. (We accept documentary evidence issued by the country of origin, however, certificates not issued by either the United States or France must be accompanied by a certified translation).

Please submit the following documents with 1 photocopy of each identification document:

Step D. Payment by French bank Euro check (chèque de banque, in Euros payable in France) made to the order of: U.S. Embassy. We do NOT accept personal chèques certifiés or personal checks. (See fees and payment methods.)
Step E. You MUST provide a return envelope. Please provide ONE of the following:
- A self-addressed Lettre Suivie 500g, or
- A self-addressed Colissimo pret-a-envoyer 1kg, or
- A self-addressed Chronopost envelope (PDF – 222 KB) for France métropolitaine for the return of passports. Please keep top copy of delivery information with bar code and tracking number for your records (one envelope can contain up to 6 passports).
